1. Application form duly accomplished & sworn/notarized. Download
2. Lumber Supply Contract/Agreement from legitimate suppliers/subsisting lumber dealer. (not required if the applicant is a mini-sawmill permittee)
3. Mayor's Permit/Business Permit and DTI Certificate (Single PDF File)
4. Annual Business Plan
5. Latest Income Tax Return
6. Pictures of the Establishment inspected/verified by CENRO concerned duly subscribed/sworn  
7. Ending stocked inventory report duly subscribed/sworn  
8. Proof of ownership of the lumberyard or consent/agreement with the owner  
9. Summary reports showing the monthly lumber purchases, production, disposition/sales ending inventory report and other relevant information within the tenure of the permit duly attested by CENRO concerned.

The Online Lumber Dealer Registration and Monitoring System is developed in order to:

  • Standardize and streamline the process flow regionwide
  • Strengthen the protection and conservation of naturally grown timber
  • Improve access to service delivery and efficient processing of lumber dealer application
  • Update statistics of all log/lumber supply contracts and aid the analysis of wood volume available versus the wood requirement
  • Minimize face-to-face transactions between and among the clients and DENR personnel
